Micro-Laparoscopy Saves 24-Day-Old Infant from Catastrophic Midgut Volvulus

A 24-day-old infant was presented with sudden-onset green bilious vomiting and severe abdominal tenderness. Emergent radiological series identified malrotation of the midgut complicated by 360-degree clockwise volvulus—an acute neonatal emergency where mesenteric vascular twisting threatens total bowel ischemia.
Rather than a large disruptive laparotomy incision, Dr. Raghul M deployed 3mm micro-laparoscopic instruments. Under high-definition magnification, the twisted bowel loops were gently counter-rotated and untwisted.
Congenital peritoneal Ladd’s bands compressing the duodenum were carefully divided, the mesenteric root was widely broadened to permanently prevent re-twisting, and an incidental prophylactic appendectomy was executed.
